Kindly follow these steps
1. Obtain a referral
To expedite the scheduling process, we strongly recommend you obtain a referral and authorization from your physician or health care provider, which can be faxed to us at (213) 462-3636, along with your most recent visit note, relevant labs, and/or imaging.
Medicare and PPO members are requested to please do the same.
2. Request an Appointment
Please call our office at (213) 460-ENDO (3636) to schedule your appointment and have your insurance information and primary care physician's information available.
3. Complete forms ahead of time
Please complete our patient forms by clicking the green button below. This step allows us to check insurance eligibility and ensure we have all the necessary information prior to your appointment.
If possible, please provide information on your government issued ID, insurance card, and prescription benefits card (if provided by your insurance company).
4. Review Clinic Policies
Please review our Clinic Policies, Information, & Procedures carefully to ensure our clinic is the right fit for you.
5. Confirm your Appointment
